Course Description: Students will work with primary care providers at SFGH. They will serve as health coaches to patients to provide focused counseling on diet, exercise and recommended preventive care. They will also provide patient education on diabetes and hypertension. They will sit in on patient visits and will 'close the loop' at the end of the visit by using the ask-teach-ask technique. They will also help with patient follow up when appropriate.
